Role Purpose:

Under the general guidance of the Intact Services Team Supervisor, this position involves providing specialized, short-term child welfare case management services to families facing significant challenges. The role includes assessing safety and risk factors that affect child welfare, facilitating the development of safety plans, and identifying the service needs of parents. Additionally, the case manager will create and implement service plans, conduct follow-ups with families to ensure proper connections with service providers, and maintain comprehensive case documentation.

Key Responsibilities:

  • Conduct in-person meetings with children, biological parents, and foster or relative caregivers as mandated.
  • Organize and lead necessary Child and Family Team meetings, ensuring active participation from families.
  • Observe and assess parent-child interactions as required.
  • Provide transportation for clients when necessary.
  • Secure necessary consents from families and service providers.
  • Assist families in accessing community resources.
  • Engage in ongoing training related to DCFS requirements.
  • Offer counseling and support services to families.
  • Refer families and children to additional services as needed.

Qualifications:

  • Bachelor's degree in social work or a related field from an accredited institution, along with one year of relevant social welfare experience.
  • Experience in case management is essential.
  • Bilingual proficiency is preferred.
  • Must obtain and maintain First Aid and CPR certification.
  • Possession of a valid driver's license and current auto insurance is required.
  • Ability to transport clients as needed.
  • Must secure DCFS Licensure within 60 days of employment.
  • Must achieve passing scores on required examinations.
  • Proficiency in Microsoft Office is necessary.
