We are currently seeking a dedicated individual to enhance our Case Management team. The BBO Case Managers Level III will focus on supporting our most vulnerable pregnant patients. This role involves collaboration with medical professionals and nursing staff to identify potential clients, assess their needs, and provide essential support services.

Key Responsibilities
  • Deliver case management services, coordinating primary care and social services for a caseload of at least 55 prenatal clients.
  • Conduct face-to-face follow-ups, complete risk assessments, and manage referrals and home visits.
  • Ensure quality case management by meeting program goals set by Erie and funding organizations.
  • Maintain accurate records of client interactions, including transit card distribution and incentive tracking.
  • Administer developmental screenings and collect participant surveys as required.
  • Perform additional duties as assigned.

Qualifications

Education
  • Master's degree in Social Work (MSW) or a related field is required.

Skills and Experience

Required:
  • Bilingual proficiency in English and Spanish, with cultural competency.
  • Valid Illinois driver's license and automobile insurance for local travel.

Preferred:
  • At least two years of experience in health, social, or community services.
