Position Overview:

The Recovery Support Specialist plays a crucial role in delivering peer support services to individuals grappling with substance use disorders (SUD). This position is dedicated to aiding patients in their journey towards sustained recovery. The specialist facilitates access to recovery-focused treatment options, resources, and community support groups, thereby bridging the fragmented treatment landscape into a more integrated care model.

Key Responsibilities:

  • Act as a vital link between the recovery community, addiction treatment services, healthcare providers, and the patient's social network to enhance connectivity across various care systems.
  • Engage with patients diagnosed with SUD who have been referred for support.
  • Conduct pre-discharge meetings with patients to ensure a smooth transition to subsequent treatment stages, including performing initial assessments and collaborating with clinical teams to set patient goals.
  • Work in tandem with treatment teams to deliver evidence-based interventions tailored to the needs of patients with SUD.
  • Assist patients in navigating healthcare and social service systems, providing accompaniment to appointments and meetings as necessary.
  • Offer transportation to clients for attending essential meetings and appointments.
  • Educate patients about addiction while employing motivational techniques to encourage their commitment to treatment.
  • Provide peer counseling, supporting patients in defining their treatment objectives and maintaining recovery.
  • Identify and address obstacles to recovery, serving as a role model and advocate for patients.
  • Engage in proactive outreach to promote participation in recovery-oriented and self-help groups.
  • Facilitate access to treatment for SUD and co-occurring disorders, establishing efficient systems to identify treatment needs and appropriate care levels.
  • Contribute to data collection efforts for quality assurance and program evaluation.
  • Collaborate with patients and care teams to develop relapse prevention and intervention strategies.
  • Monitor patient progress and outcomes using a tracking system.
  • Ensure effective communication among patients, primary care providers, and external healthcare professionals while maintaining confidentiality.
  • Keep accurate and current records on all patients, reviewing caseloads with supervisors to focus on new and challenging cases.

Qualifications:

  • Certification as a Recovery Coach or Peer Specialist is required.
  • A minimum of three years in sustained recovery is essential.
  • Possession of reliable transportation and comprehensive insurance coverage.
  • Strong interpersonal, facilitation, and leadership capabilities.
  • Ability to work autonomously and collaboratively within a multidisciplinary team of healthcare professionals.
  • Advocacy skills coupled with knowledge of community resources and local recovery networks.
  • Comfort with diverse recovery pathways and a commitment to a patient-centered approach.
  • Experience in both medical and social service environments.
  • Clinical expertise in motivational enhancement strategies.
  • Capacity to provide constructive feedback in a non-judgmental manner.
  • Flexibility to adapt to changing circumstances and needs.
  • Strong problem-solving skills, organizational abilities, and goal-oriented mindset.
  • Excellent communication skills, both written and verbal.
  • Proficient in using the Internet and Microsoft Office applications.
  • Ability to manage confidential information responsibly.
