Position Overview:

The Recovery Support Specialist plays a crucial role in delivering peer-based recovery assistance to individuals facing substance use challenges. This position is dedicated to guiding patients towards long-term recovery by facilitating access to treatment options, community resources, and support groups focused on recovery. The specialist acts as a bridge between fragmented treatment services, promoting a comprehensive care approach that supports patients throughout their recovery journey.

Key Responsibilities:

  • Serve as a vital link among the recovery community, addiction treatment providers, healthcare professionals, and the patient's personal network to enhance connectivity across various care systems.
  • Engage with individuals identified as having substance use disorders (SUD) to provide support and resources.
  • Conduct pre-discharge meetings with patients to assess needs, review SUD history, and collaboratively set recovery goals with clinical teams.
  • Collaborate with treatment teams to deliver evidence-based interventions tailored to the needs of patients with SUD.
  • Assist patients in navigating healthcare and social services, including accompanying them to appointments when necessary.
  • Provide transportation to clients for attending essential meetings and appointments.
  • Educate patients about addiction and employ motivational techniques to encourage treatment engagement.
  • Offer peer counseling to help patients establish and achieve their recovery objectives.
  • Identify and address barriers to recovery, serving as a role model and advocate for patients.
  • Conduct outreach initiatives to promote participation in recovery-oriented and self-help groups.
  • Facilitate access to treatment for SUD and co-occurring disorders, ensuring rapid identification of treatment needs and appropriate care levels.
  • Support data collection efforts for quality assurance and program evaluation.
  • Develop personalized relapse prevention and intervention strategies in collaboration with patients and care teams.
  • Monitor patient progress and evaluate outcomes using a patient tracking system.
  • Ensure effective communication among patients, primary care providers, consulting psychiatrists, and external providers while maintaining confidentiality.
  • Maintain accurate and current records on all patients, reviewing caseloads weekly with supervisors to focus on new and challenging cases.

Qualifications:

  • Certification as a Recovery Coach or Peer Specialist is required.
  • A minimum of three years in sustained recovery is essential.
  • Possession of reliable transportation and comprehensive insurance coverage.
  • Strong interpersonal, facilitation, and leadership abilities.
  • Capacity to work independently and collaboratively within a multidisciplinary team of healthcare professionals.
  • Advocacy skills and familiarity with community resources and local recovery networks.
  • Open-mindedness towards diverse recovery pathways and a commitment to a patient-centered approach.
  • Comfortable working in both medical and social service environments.
  • Clinical proficiency in motivational enhancement strategies.
  • Able to provide constructive feedback in a non-judgmental manner.
  • Flexibility to adapt to changing needs and circumstances.
  • Strong problem-solving and organizational skills, with the ability to maintain effective relationships with patients and staff.
  • Excellent communication skills, both written and verbal.
  • Proficient in using the Internet and Microsoft Office applications.
  • Ability to manage confidential information responsibly.
