Job Overview

The Family Health Center is dedicated to providing comprehensive medical and behavioral health services to individuals facing homelessness. Our multidisciplinary team operates from a modern clinic and various outreach locations within the community.

Position Summary:

The Recovery Coach plays a vital role in delivering peer support services to individuals dealing with substance use disorders (SUD). This position is essential in guiding patients towards achieving long-term recovery.

  • Facilitates access to recovery-focused treatment options, community resources, and support groups.
  • Collaborates with the healthcare team to ensure coordinated and compassionate care.
  • Maintains effective communication with healthcare professionals, community organizations, and support service providers.
  • Utilizes knowledge of community dynamics to address recovery barriers and tailor health communications effectively.
  • Engages in outreach activities and provides support at community events.
  • Conducts home, shelter, or community visits as necessary.

Qualifications:

  • High School Diploma or GED is required; an Associate's Degree in Human Services or a related field is preferred.
  • Completion of the Recovery Coach Academy is mandatory.

Experience:

  • Personal experience with substance use disorder is essential.
  • Preferred two years of experience in healthcare or substance abuse support programs.
  • Familiarity with community resources is necessary.
  • Proficiency in computer skills is required.

Licensure and Skills:

  • Certification as an Addiction Recovery Coach (CARC) is required or must be obtained within six months of hiring.
  • Local travel is necessary; a valid driver's license and access to transportation are required.
  • CPR/BLS certification is mandatory.

Interpersonal Skills and Expectations:

  • Dedication to providing healthcare to underserved populations.
  • Flexibility in developing innovative health service delivery methods.
  • Ability to handle sensitive information with confidentiality.
  • Strong communication skills, both verbal and written, with various stakeholders.
  • Bilingual abilities are preferred but not required.
  • Capability to work independently with minimal supervision while managing responsibilities effectively.
  • Awareness of available resources within the health center to support patient needs.
  • Commitment to delivering cost-effective, patient-centered, and culturally competent care.
  • Adherence to HIPAA policies regarding personal health information.

Reporting Structure:

This position reports directly to the Lead Case Manager.
