Position Overview:
As a key member of the healthcare team, the Patient Coordinator plays a vital role in supporting patients within the Adult Cystic Fibrosis Program. This position is essential for ensuring seamless communication and coordination between patients and healthcare providers.

Responsibilities:
1. **Patient Support:**
- Provide comprehensive assistance to patients requiring advanced administrative services related to their treatment.
- Facilitate pre-authorizations for hospital admissions and coordinate with relevant departments to ensure timely patient care.

2. **Communication Management:**
- Serve as the primary liaison for patients, ensuring they receive necessary information regarding their treatment and appointments.
- Monitor and remind patients of their scheduled hospital visits and procedures.

3. **Appointment Coordination:**
- Schedule and manage appointments for interventional services and follow-up care.
- Ensure that all medical records and imaging are available for new patient visits.

4. **Data Management:**
- Maintain accurate records of patient interactions and treatment plans in the electronic medical record system.
- Regularly update lab results and coordinate with home nursing services to monitor patient progress.

5. **Collaboration:**
- Work closely with the healthcare team to ensure all patient needs are met and that they receive high-quality care.
- Participate in team meetings to discuss patient care strategies and improvements.

Qualifications:
- High school diploma or equivalent required.
- Completion of an accredited Medical Assistant program or equivalent training.
- A minimum of one year of experience in a healthcare setting, preferably as a Medical Assistant or Certified Nursing Assistant.
- CPR/BLS certification is mandatory.
