Community Living Support Specialist

At Living Opportunities, we are seeking a highly skilled and compassionate Community Living Support Professional to join our team. As a key member of our Supported Living Services department, you will play a vital role in supporting individuals with disabilities to live independently in their own homes.
Key Responsibilities:- Conduct comprehensive support needs assessments for individuals with disabilities to identify opportunities for increased physical and social integration in the community.
- Develop and implement individualized support plans that promote choice, control, and independence for individuals with disabilities.
- Provide assistance, training, and monitoring to support individuals with disabilities in maintaining their daily living skills, including personal care, meal preparation, and household management.
- Collaborate with healthcare professionals, family members, and other support staff to ensure seamless care coordination and support.
- Maintain accurate and detailed records of individual support plans, progress, and outcomes.
- High school diploma or equivalent required; associate's or bachelor's degree in human services or related field preferred.
- Minimum of 1 year of experience working with individuals with disabilities, preferably in a supported living setting.
- Excellent communication, interpersonal, and problem-solving skills.
- Ability to work independently with minimal supervision and as part of a team.
- Valid driver's license and reliable transportation required (if applicable).
