Job Title: Social Worker - Community Inclusion Specialist

We are seeking a highly motivated and compassionate Social Worker to join our team as a Community Inclusion Specialist. As a Community Inclusion Specialist, you will work with adults with developmental disabilities and/or autism to create and facilitate outings that promote independence and confidence.

Key Responsibilities:
  • Supervise and oversee participants in the community on a daily basis to achieve their personal goals and take part in activities and/or events.
  • Meet the basic needs of our participants while in the community, including assistance with Activities of Daily Living (ADLs) as required.
  • Implement Individual Support Plans (ISPs) and behavior plans as needed.
  • Maintain knowledge of participants' ISPs and personal needs to work with them in a meaningful and productive way.
  • Track receipts and expenditures for reconciliation.
  • Complete all required paperwork, including case notes on a regular and timely basis.
  • Drive personal vehicle or use public transportation to assist individuals in accessing the community.
  • Attend to basic first aid when needed.
Requirements:
  • One year of experience working with people with developmental disabilities.
  • High School Diploma/GED, Bachelor's degree preferred.
  • Basic computer skills using Microsoft Products.
  • Valid driver's license, access to a vehicle, and clean driving record.
