Community Inclusion Specialist

We are seeking compassionate and dedicated Social Workers to join our team as Community Inclusion Specialists (CIS). In this role, you will play a vital part in supporting adults with developmental disabilities and/or autism in achieving their personal goals and living fulfilling lives within the community.

Your Responsibilities:
  • Provide direct support to individuals with developmental disabilities, assisting them in participating in community activities and events aligned with their Individual Support Plans (ISPs).
  • Ensure the well-being of participants by addressing their basic needs and providing assistance with Activities of Daily Living (ADLs) as required.
  • Implement ISPs and behavior plans, tailoring your approach to meet each individual's unique needs and goals.
  • Maintain accurate records, including case notes and expenditure tracking, ensuring compliance with organizational policies and procedures.
  • Utilize public transportation or personal vehicles to transport individuals safely and efficiently throughout the community.
  • Respond to basic first aid situations as needed, prioritizing the safety and well-being of participants.
Qualifications:
  • One year of experience working with individuals with developmental disabilities is required.
  • A High School Diploma or GED is essential; a Bachelor's degree in Social Work or a related field is preferred.
  • Proficiency in basic computer skills using Microsoft Office Suite.
  • Possession of a valid driver's license, reliable transportation, and a clean driving record.
