Community Living Support Specialist

Job Summary:
We are seeking a highly motivated and compassionate Community Living Support Professional to join our team at Living Opportunities. As a key member of our Supported Living Services department, you will play a vital role in supporting individuals with disabilities to live independently in their own homes in the community.
Key Responsibilities:
- Conduct comprehensive support needs assessments for individuals and develop strategies to meet those needs.
- Identify opportunities for individuals to achieve increased physical and social integration in the community.
- Implement individual support plans, providing assistance, training, and monitoring as identified in each person's plan.
- Provide support for individuals with disabilities, promoting independence, integration, choice, and control.
- Maintain accurate records and documentation, including data for training and support activities, house logs, medication and medical records, incident reports, menus, safety and cleaning checklists, and emergency evacuation records.
- Collaborate with individuals, family members, case managers, physicians, and other support providers to ensure seamless support services.
- Report incidents and accidents in writing, using the agency-mandated Incident Report form.
Requirements:
- High school diploma or GED and some college-level coursework or para-professional certification (e.g., CNA).
- Associate's or bachelor's degree in human services or a related field is preferred.
- Ability to work independently with minimal supervision.
- Effective communication, time management, and organizational skills.
- Ability to maintain confidentiality and adhere to agency policies and procedures.
