Nutrition Services Coordinator at Mount Sinai Health System

The Nutrition Services Coordinator is responsible for delivering high-quality nutritional care and education to patients and their families, guided by the Director of Nutrition Services. This role involves overseeing departmental initiatives and educational programs while maintaining a high level of clinical expertise in the designated area. The Coordinator recommends suitable nutritional therapies tailored to patients' needs and serves as a resource for clinical staff, providing feedback to support their professional growth. Compliance with regulatory agency guidelines, as well as adherence to departmental and hospital standards, policies, and procedures, is essential.

Key Responsibilities
  • Assess the nutritional status of patients by analyzing relevant information from electronic medical records and collaborating with the interdisciplinary healthcare team. Utilize knowledge of nutrition screening and assessment to identify the specific nutritional needs of diverse patient populations.
  • Document significant findings in the medical record according to established policies.
  • Deliver effective nutrition counseling to patients and their families as necessary.
  • Coordinate departmental projects, including nutrition-related training sessions, interdepartmental educational programs, and presentations.
  • Create, develop, and regularly update nutrition education materials to address the unique needs of specific patient groups.
  • Engage in quality assurance monitoring and reporting for departmental performance improvement initiatives.
  • Supervise dietetic interns, develop educational modules, instruct them on required competencies for their rotations, and complete evaluations.
  • Review and assess the effectiveness of nutrition care plans, recommending adjustments as needed. Employ professional judgment to adapt guidelines, protocols, and standards of care to meet the evolving needs of patients.
  • Participate in practice-specific research to enhance departmental programs.
  • Join interdisciplinary rounds as required, effectively communicating with healthcare team members. Act as a resource for all hospital staff regarding nutrition requirements for specific patient populations.
  • Assist with discharge planning and home care needs as appropriate.
  • Engage in community-focused educational initiatives and external speaking engagements to promote services and knowledge.
  • Facilitate department journal clubs.
  • Perform other duties as assigned.
Qualifications

Essential Requirements:
  • Bachelor's degree from an accredited institution; a Master's degree is preferred.
  • 3-5 years of relevant experience.
  • Registered Dietitian (RD) credential from the Commission on Dietetic Registration is required.
  • Certified Dietitian Nutritionist (CDN) from the NYS Department of Education Office of the Professions is preferred but not mandatory.
  • Experience with complex gastrointestinal issues and nutrition support (PN) is preferred.
  • Experience in areas such as Intestinal Failure, Transplant, and Rehabilitation is preferred.
  • Advanced credentials such as Certified Diabetes Educator (CDE), Certified Nutrition Support Clinician (CNSC), or specialties in Pediatric, Renal, or Oncology are preferred but not required.
