Position Overview

NYC Health + Hospitals/Community Care is dedicated to extending healthcare services beyond traditional settings, ensuring that patients receive the necessary support in their communities. With a workforce of over 600 committed professionals, Community Care specializes in home care, community coordination, and management for individuals with chronic health issues across various boroughs.

Role Purpose

The Registered Dietitian will deliver comprehensive nutrition care through assessment, diagnosis, intervention, monitoring, and evaluation for patients under the New York City Health + Hospitals Certified Home Health Agency. This role involves ensuring that all nutrition-related care is documented and executed in line with the Agency's established policies and regulatory standards.

Key Responsibilities

1. Nutritional Assessments: Conduct thorough evaluations of patients' dietary needs, engaging with patients and their families to gather information on food habits, preferences, and treatment objectives. Analyze medical records to inform nutritional assessments.

2. Care Planning: Develop and document individualized nutritional assessments and care plans that align with patients' diagnoses and physician orders, ensuring specificity and relevance.

3. Patient Monitoring: Regularly review and discuss dietary needs with patients, providing guidance on therapeutic diets and modifications for home preparation.

4. Evaluation of Treatments: Monitor patients for potential food/drug interactions and assess the effectiveness of nutrition therapies. Assist in creating nutritional analyses for therapeutic menus.

5. Documentation and Reporting: Observe and document patient responses to treatments, reporting any changes in condition to the appropriate healthcare professionals.

6. Interdisciplinary Coordination: Collaborate with other healthcare professionals to maintain continuity of care, participating in patient care conferences as necessary.

7. Compliance and Quality Assurance: Ensure that all patient care documentation is accurate and timely, adhering to Agency policies and regulatory requirements.

8. Case Management: Fulfill case assignments and participate in interdisciplinary reviews to optimize clinical outcomes for patients.

9. Patient Care Concerns: Promptly escalate any patient care issues to the appropriate nursing leadership to mitigate risks of patient decline.

10. Quality Improvement Participation: Engage in quality assurance initiatives and conduct evaluations of dietitians in accordance with regulatory standards.

Working Conditions

This role involves daily home visits, which may require travel via walking, driving, or public transportation. The position may also necessitate climbing stairs and regular communication with the office.

Qualifications

Licenses/Certifications: Must possess a New York State License as a Registered Dietitian, current registration with the NYS Department of Education in Nutrition, and certification in Basic Life Support (BLS) procedures including CPR.

Education: A Bachelor's Degree or higher in dietetics or a related field accredited by the Academy of Nutrition and Dietetics is required.

Experience: A minimum of one year of post-licensure experience as a Registered Dietitian is necessary.

Skills: Proficiency in the essential skills of a Registered Dietitian is expected, with familiarity in Home Care regulations being advantageous but not mandatory.

Technology Proficiency

Familiarity with various equipment and software, including smartphones, laptops, EPIC systems, and Microsoft Office Suite, is preferred.
