Job Summary

We are seeking a highly skilled and experienced Supervisor of Social Services to lead our team of Case Managers and Housing Specialists in a 68-bed transitional setting. The successful candidate will be responsible for providing leadership and guidance to our team, developing and maintaining supportive services, and ensuring that service delivery and documentation meet contractual agreements.

Key Responsibilities
  • Provide administrative and clinical supervision to Case Managers, including documented individual supervision meetings, ongoing feedback, and participation in case conferencing and service planning.
  • Develop and maintain on-site supportive services, including peer support groups, relapse prevention, housing and ADL groups, and health education.
  • Provide individual substance abuse and mental health counseling as appropriate.
  • Provide training and consultation to Case Managers on co-occurring disorders and evidence-based practices.
  • Utilize and monitor the AWARDS database system, conduct internal chart reviews, and prepare for funder audits.
  • Ensure that service delivery and documentation meet contractual agreements.
  • Act as a liaison to the HASA-EPU around admissions and room closures.
  • Monitor daily rosters and provide information for the Monthly Occupancy Report.
  • Monitor housing placements and participate in the HASA monthly reporting process.
  • Participate in consumer intake, orientation, assessment, and service planning process, and monitor consumer progression and/or regression toward service plan goals.
  • Develop and maintain relationships with community-based medical, mental health, and substance abuse service providers.
  • Work as a member of the social service team in the development of on-site and off-site recreational, socialization, educational, and skill-building activities with a view towards community inclusion and integration.
  • Adhere to on-call protocol to facilitate crisis intervention, address emergencies, and other situations that require immediate action.
  • Participate in team meetings, case conferencing, division meetings, and trainings.
  • Participate in the Services for the Underserved Quality Assurance activities, including incident documentation, management, and reporting.
  • Coordinate with the Maintenance Supervisor around room/apartment/building inspections, fire drills, apartment/room repairs, heat and hot water, habitability, and consumer safety issues.
  • Reports to the Regional Director of Homeless Services in the absence of the Site Director.
Qualifications
  • Must be a Licensed Master Social Worker (LMSW).
  • CASAC or CASAC-T strongly preferred.
  • Minimum of three (3) years of clinical experience with individuals with HIV/AIDS, substance abuse issues, mental illness, and history of homelessness required.
  • Minimum of two years of supervisory experience required.
  • Strong written and verbal communication skills required.
  • Knowledge of the NYC HRA HASA entitlement system preferred.
