Position Overview

Services for the UnderServed is seeking a Social Services Team Leader for our transitional housing program.

Schedule: Monday-Friday 9a-5p, with flexibility and on-call responsibilities.

This role presents an excellent opportunity for recent graduates in Social Work looking to advance their careers. A Licensed Master Social Worker (LMSW) credential is required. The compensation range is between $62,000 - $67,000 annually.

The Social Services Team Leader will oversee a dedicated team of four Case Managers and Housing Specialists within a 68-bed transitional environment, assisting individuals affected by HIV/AIDS in achieving permanent housing solutions.

Key Responsibilities:
  • Provide leadership and direction to Case Managers on matters related to mental health, substance abuse, crisis intervention, and consumer incidents.
  • Administer clinical supervision to Case Managers, which includes conducting documented individual supervision meetings, offering ongoing performance feedback, participating in case conferences, and evaluating documentation.
  • Develop and maintain on-site supportive services, including peer support groups, relapse prevention initiatives, and health education programs.
  • Offer individual counseling for substance abuse and mental health as necessary.
  • Deliver training and consultation to Case Managers regarding co-occurring disorders and evidence-based practices that enhance consumer wellness.
  • Utilize and monitor the AWARDS database system, conduct internal chart reviews, and prepare for audits by funders.
  • Ensure compliance with service delivery and documentation as per contractual obligations.
  • Act as a liaison with relevant agencies regarding admissions and room closures.
  • Monitor daily occupancy and provide data for monthly reports.
  • Participate in consumer intake, orientation, assessment, and service planning, while tracking consumer progress towards goals.
  • Build and maintain relationships with community-based medical, mental health, and substance abuse service providers.
  • Collaborate with the social service team to create recreational, educational, and skill-building activities that promote community inclusion.
  • Adhere to on-call protocols for crisis intervention and emergency situations.
  • Engage in team meetings, case discussions, and training sessions.
  • Contribute to quality assurance activities, including incident documentation and reporting.
  • Coordinate with maintenance staff regarding inspections and safety issues.
  • Report to the Regional Director of Homeless Services in the absence of the Site Director.
  • Possess a working knowledge of various software applications, including Microsoft Office Suite.
  • Evening and weekend hours may be necessary.
  • Perform additional duties as assigned by the Site Director.
Qualifications:
  • Must hold a Licensed Master Social Worker (LMSW) credential.
  • CASAC or CASAC-T certification is strongly preferred.
  • A minimum of three years of clinical experience with individuals facing HIV/AIDS, substance abuse, mental health challenges, and homelessness is required.
  • At least two years of supervisory experience is necessary, along with strong communication skills.
  • Experience in residential settings is highly preferred, along with skills in group work, community outreach, and program development.
