Services for the UnderServed is seeking a Social Services Team Leader for our transitional housing program.
Schedule: Monday-Friday 9 AM - 5 PM, with flexibility and on-call responsibilities.
This is an excellent opportunity for a recent graduate in Social Work looking to advance their career. LMSW is required. Compensation ranges from $62,000 to $67,000 annually.
The Social Services Team Leader will oversee a dedicated team of four Case Managers and Housing Specialists within a 68-bed transitional environment, assisting individuals affected by HIV/AIDS in achieving permanent housing.
Key Responsibilities:- Provide leadership and direction to Case Managers regarding mental health and substance abuse challenges, including crisis intervention and consumer incidents.
- Administer clinical supervision to Case Managers, encompassing documented individual supervision meetings, performance feedback, participation in case discussions, and staff development.
- Establish and maintain on-site supportive services, such as peer support groups, relapse prevention, and health education.
- Deliver individual counseling for substance abuse and mental health as necessary.
- Offer training and consultation to Case Managers on co-occurring disorders and evidence-based practices that promote consumer wellness.
- Utilize and monitor the AWARDS database system, conduct internal chart reviews, and prepare for audits.
- Ensure compliance with service delivery and documentation according to contractual obligations.
- Act as a liaison for admissions and room closures.
- Monitor daily rosters and assist in the Monthly Occupancy Report.
- Oversee housing placements and contribute to monthly reporting processes.
- Engage in consumer intake, orientation, assessment, and service planning, monitoring progress towards goals.
- Build and maintain relationships with community-based medical, mental health, and substance abuse service providers.
- Collaborate with the social service team to develop recreational, educational, and skill-building activities that promote community inclusion.
- Adhere to on-call protocols for crisis intervention and emergency situations.
- Participate in team meetings, case discussions, and training sessions.
- Engage in quality assurance activities, including incident documentation and reporting.
- Coordinate with maintenance regarding inspections and repairs.
- Report to the Regional Director of Homeless Services in the absence of the Site Director.
- Possess working knowledge of various software programs including Microsoft Office Suite.
- Evening and weekend hours may be necessary.
- Perform additional duties as assigned by the Site Director.
- Must possess a Licensed Master Social Worker (LMSW) credential.
- CASAC or CASAC-T certification is strongly preferred.
- A minimum of three years of clinical experience with individuals facing HIV/AIDS, substance abuse issues, mental illness, and homelessness is required.
- At least two years of supervisory experience is necessary, along with strong communication skills.
- Experience in a residential setting is highly preferred, along with skills in group work, community outreach, and program development.
