Position Overview

The Head of Social Services is tasked with the strategic planning, organization, development, and management of the Social Services Department, ensuring compliance with all applicable federal, state, and local regulations, as well as the established policies and procedures of Med-Metrix. This role provides professional leadership to the Social Work team across the organization, ensuring that the emotional, social, and medically related needs of patients are consistently met and upheld.

Key Responsibilities

  • Ensure adherence to regulations governing social work practices and transition planning.
  • Facilitate collaboration among inpatient and outpatient social workers/care managers to promote seamless care transitions.
  • Oversee all medical transport requirements within the organization.
  • Conduct financial analysis to support departmental operations.
  • Manage productivity and workload effectively.
  • Guarantee compliance with New York State Department of Health and Joint Commission standards, including mandatory requirements and performance evaluations.
  • Follow administrative and departmental policies and procedures diligently.
  • Develop comprehensive business and action plans.
  • Maintain positive relationships with labor representatives.
  • Address conflicts among staff in a constructive manner and seek HR consultation when necessary.
  • Manage employee attendance and punctuality effectively.
  • Provide balanced assessments of individual strengths and developmental needs.
  • Align employee strengths with job requirements and make appropriate assignments.
  • Deliver timely feedback aimed at enhancing performance.
  • Facilitate professional growth through stretch assignments and training opportunities.
  • Encourage stakeholder involvement and participation.
  • Proactively address patient/family and staff satisfaction issues.
  • Identify and manage length of stay (LOS) challenges.
  • Develop and assess case management strategies for high-risk patients.
  • Utilize data to enhance departmental performance.
  • Collaborate with other leaders to ensure integrated Case Management/Social Work services.
  • Identify and oversee clinical revenue enhancement initiatives.
  • Support programs that provide educational experiences for staff and students.
  • Actively contribute to the development and revision of case management policies.
  • Communicate policy changes effectively to staff.
  • Perform additional related duties as assigned.
  • Safeguard and manage patient health information in compliance with HIPAA standards.
  • Limit access to protected health information to the minimum necessary for job functions.
  • Understand and adhere to Information Security and HIPAA protocols consistently.
Qualifications
  • Master's Degree in Social Work.
  • At least 5 years of experience in hospital social work.
  • A minimum of 5 years in a leadership role within social work.
  • Licensed Master Social Worker (LMSW) in New York State.
  • Required screening certification.
  • Comprehensive knowledge of community resources and regulatory requirements in case management/social work.
  • Strong understanding of CMS Conditions of Participation related to transition management.
  • Familiarity with HIPAA and privacy regulations.
  • Knowledge of healthcare laws and standards.
  • Exceptional written and verbal communication skills.
  • Strong leadership, managerial, and interpersonal abilities.
  • Effective problem-solving skills with the capacity to engage with various stakeholders diplomatically.
  • Ability to follow instructions in various formats effectively.
Working Conditions
  • Physical Demands: The role requires occasional movement within the work area, sitting, performing manual tasks, and operating office equipment. Light lifting (up to 15 pounds) may be necessary.
  • Mental Demands: The employee must be capable of following directions, collaborating with others, and managing stress effectively.
  • Work Environment: The position is based in a well-lit and ventilated office setting, subject to frequent interruptions. There is minimal exposure to infectious diseases and hazardous materials. Adherence to hospital safety standards is essential.
