Position Overview:
As a Social Work Assistant 3, you will engage in a range of case management and service coordination tasks for an assigned caseload. Your responsibilities will include:

Key Responsibilities:
  • Conducting interviews with individuals, families, and relevant parties to collect background information and prepare comprehensive case histories.
  • Determining eligibility for various services and advocating for individuals to help them access necessary resources.
  • Collaborating with interdisciplinary treatment teams to develop and implement effective treatment and service plans.
  • Coordinating services across different agencies, programs, and providers to ensure comprehensive support for individuals.
  • Following up to ensure that the needs of individuals are met and that they receive ongoing support.

Eligibility Requirements:
To qualify for this position, candidates must meet specific educational and experience criteria, including:
  • A bachelor's degree or higher in a human service area, along with relevant post-bachelor's experience.
  • Possession of a master's degree in social work or a bachelor's degree in social work with one year of post-bachelor's social work experience.

Important Notes:
All candidates must not be listed as excluded individuals or entities on any Federal or State Medicaid and Medicare exclusion lists. If appointed, candidates may be required to enroll as a Medicare provider and obtain a National Provider Identifier (NPI) number.
