Family Support Services Coordinator

We are thrilled to announce an opportunity for a dedicated and skilled Family Support Services Coordinator to become a vital member of our team. Homes for the Homeless (HFH) is on the lookout for a qualified Family Support Services Coordinator to work with families experiencing homelessness. In this key position, you will significantly contribute to our mission by delivering mental health and supportive services to families in need. The ideal candidate will enhance our current offerings by conducting comprehensive assessments of families' long-term aspirations during the intake process and collaborating closely with case management personnel to establish realistic objectives and foster connections within the community to ensure successful outcomes for families. Homes for the Homeless operates shelters for families facing homelessness in New York City. Our family residences provide more than just shelter; they offer access to a variety of onsite programs designed to empower families during their stay. Many of these programs are also accessible to local community members. Our goal is to create a lasting positive impact, enabling families to thrive long after they transition from our shelters.

KEY RESPONSIBILITIES
  • Conduct thorough bio-psychosocial evaluations with each family to identify strengths and service requirements.
  • Oversee, guide, and coordinate social services and counseling support, ensuring timely and appropriate interventions.
  • Focus on effective delivery of social services to clients, integrating best practices such as trauma-informed and family-centered care.
  • Track and document client progress towards established service plan goals.
  • Ensure families receive comprehensive support within the shelter and refer them to counseling or additional services as needed.
  • Assist the Family Services department by collaborating with Case Managers and supporting staff in managing challenging client situations.
  • Enhance connections to mental health, educational, and community-based services.
  • Strengthen the overall permanency outcomes for families with children in shelter.
  • Manage record-keeping, data collection, and evaluation of service effectiveness for families.
  • Assist with unit inspections as required.
  • Support the Family Services department by providing training sessions.
  • Promote and exemplify best practices for Family Services staff.
QUALIFICATIONS
  • A master's degree in Social Work or Mental Health Counseling is required.
  • Applicants must possess an LMSW (Licensed Master Social Worker) or LMHC (Licensed Master Health Counselor) license in New York State.
  • Familiarity with substance use disorders, addiction treatment services, domestic violence, child abuse, trauma, and mental health conditions is essential.
  • Experience working with homeless and vulnerable populations is advantageous.
  • Bilingual candidates are encouraged to apply.
