In your role as the Family Support Coordinator, you will be instrumental in our mission, guiding families in accessing essential services and resources tailored to their unique needs.

ABOUT HOMES FOR THE HOMELESS, INC.

Homes for the Homeless, Inc.

(HFH) is a non-profit organization dedicated to providing social services through family shelters across NYC, serving as a beacon of hope for families both in shelter and in the surrounding community.

Our on-site programs encompass a wide range of services, including childcare, early childhood education, afterschool activities, youth recreation, housing assistance, case management, parenting workshops, vocational training, support groups, and family engagement events.


KEY RESPONSIBILITIES
Maintain and organize both digital and physical records for all families in compliance with regulatory requirements.

Collaborate with families to create tailored service plans based on thorough assessments of their needs.

Facilitate referrals to external agencies for employment, education, legal, medical, and mental health services.

Regularly update case files to document family progress and ensure continuity of care.

Assist families in transitioning to permanent housing solutions.

Ensure clients maintain active public assistance cases.

Comply with all local and state regulations regarding documentation.

Conduct inspections of family units as necessary.

QUALIFICATIONS
Bachelor's or Associate's Degree

Experience in case management is advantageous.

Proficiency in Microsoft Outlook, Word, and Excel.

Bilingual abilities in Spanish or other languages are preferred.
