Your Role in Making a Difference:

As a Case Manager, you will deliver Community Psychiatric Support and Treatment (CPST) services aimed at providing goal-oriented functional support to children and their families. Your efforts will help achieve stability and enhance daily living, personal recovery, and interpersonal relationships, fostering integration into school and community settings.

CPST services are primarily conducted in community settings for children who may struggle with traditional office environments but can benefit from home or community-based rehabilitative services. The involvement of family or caregivers is crucial in addressing the child's behavioral health needs.

Your responsibilities will include assisting children and families in identifying strategies or treatment options to alleviate the impact of symptoms, emotional disturbances, substance use, or environmental stressors affecting their functioning across various domains (home, school, community).

Examples of support may include enhancing emotional self-regulation, making healthy choices, and developing age-appropriate coping strategies.

Additionally, you will help families identify strengths, needs, resources, and natural supports, while developing goals to leverage these strengths, improve relationships, and boost self-esteem.

Providing restoration and rehabilitation services will be key to minimizing the adverse effects of behavioral health symptoms on daily living.

Examples of these services include improving life safety skills, managing healthcare appointments, and understanding medication administration for both physical and mental health conditions.

Services will be delivered in various settings, including homes, community spaces, and via telehealth. You will also participate in clinical treatment team meetings, staff meetings, and training sessions.

It is essential to maintain all documentation in accordance with the standards set by The Jewish Board and relevant regulatory agencies.

Upholding professional behaviors and ethical standards as defined by licensing boards and organizational policies is crucial.

Traveling within the community to provide services where they are most beneficial is a key aspect of this role.

Collaboration as part of an interdisciplinary therapeutic team will be vital, along with other duties as assigned.


Qualifications:
The Jewish Board seeks individuals from diverse backgrounds with the following qualifications:


A Bachelor's degree and a minimum of two years of relevant experience in children's mental health, addiction, or child welfare systems.

Experience with electronic health records and proficiency in Microsoft Office is preferred.
Bilingual Spanish/English candidates are encouraged to apply.
