Key Responsibilities- Assist clients in completing intake applications, screenings, and forms, ensuring accurate and timely documentation.
- Conduct initial risk assessments of clients and their families, identifying potential areas of concern and developing strategies for support.
- Prepare and update biopsychosocial evaluations, adhering to regulatory requirements and best practices.
- Provide instant response services to clients in crisis, ensuring their immediate well-being and safety.
- Collaborate with clients and case managers to develop and implement Independent Living Plans (ILPs), setting short-term and long-term goals for client success.
- Identify and connect clients with community resources and services, both within and outside CAMBA, to support their needs and goals.
- Follow up with clients and referral organizations to ensure progress and outcomes, making adjustments to service plans as needed.
- Act as an advocate for clients and their families, arranging for medical, psychiatric, educational, and other tests and examinations to address underlying issues.
- Facilitate case conferences with stakeholders to ensure a collaborative approach to services, promoting client stability and success.
- Provide group counseling sessions and psychoeducation to enhance social development and psychological education, fostering a supportive community among clients.
- Monitor client progress, updating service plans and goals to reflect current status and needs.
- Ensure all client charts contain required documentation, maintaining accurate and comprehensive records.
- Licensed Master of Social Work (LMSW) required.
- Must sign NYC DOH/MH attestation stating that candidate has not been the subject of a founded child abuse report.
