ABOUT THE ROLE

The Youth Assertive Community Treatment (ACT) services aim to enhance the quality of life for youth experiencing significant challenges due to mental health issues or serious emotional disturbances.

Our clinical and rehabilitative strategies focus on improving family dynamics to promote health, stability, and reintegration for youth transitioning from residential treatment or inpatient care.

The Youth ACT Team is a collaborative, multi-disciplinary group dedicated to delivering family-centered, youth-guided, and developmentally appropriate services that address the comprehensive needs of youth across various domains including family, education, health, and community.

As a Youth Family Support Specialist, you will work within a multi-disciplinary team to provide treatment and support to families and youth aged 10 to 21 facing significant behavioral health challenges and at risk of entering or returning from high-level services.

Your responsibilities will include delivering personalized services focused on clinical treatment, family education, and skills development.

You will engage with families in their homes and communities, collaborating closely with other service providers and systems that interact with the family.

Evening availability and rotating on-call coverage will be required.

KEY RESPONSIBILITIES:
  • Advocate for families within the child-serving system to promote awareness and reduce stigma.
  • Educate families on self-help techniques and group processes.
  • Provide psychoeducation to family members and caregivers.
  • Facilitate individual or group skill development for parents related to their child's behavioral health needs.
  • Teach effective coping strategies based on personal experience and assist in building community support networks.
  • Support families in developing skills to manage their child's behaviors and navigate various systems.
  • Identify culturally affirming formal and informal resources for families facing social-emotional or mental health challenges.
  • Assist families in recognizing their challenges, strengths, and goals for improvement.
  • Monitor and document family progress in accordance with agency policies.
  • Collaborate with care providers and community supports to help families achieve their goals.
  • Participate in team meetings, training sessions, and supervision.
  • Maintain documentation as per the standards set by The Jewish Board and regulatory bodies.
  • Perform services in various settings including homes and community locations.
  • Undertake additional duties as assigned.

EDUCATIONAL & TRAINING REQUIREMENTS:
  • High School Diploma or equivalent
  • Specialty credentialing in advocacy can be obtained during the probationary period
  • Lived experience in parenting a child with serious emotional disturbances or mental health challenges is preferred
  • Ability to work with diverse social, cultural, and economic groups
  • Background in mental health advocacy or education is advantageous
  • Bilingual in Spanish/English is a plus

COMPUTER SKILLS REQUIRED:
  • Experience with electronic health records and Microsoft Office

WORK ENVIRONMENT:

Most services are delivered in community settings, with limited office-based work. The role requires travel throughout New York City, carrying necessary equipment.


EXPERIENCE REQUIRED:

Bilingual in Spanish is preferred.
