Position Overview:
We are seeking dedicated and empathetic Care Coordinators to assist program participants and their guardians in navigating the healthcare system. This role involves addressing access challenges, fostering relationships with service providers, and monitoring interventions and outcomes.

Key Responsibilities:

  • Secure necessary enrollment consents from individuals or their legal guardians.
  • Conduct initial and ongoing assessments to determine appropriate care management levels.
  • Oversee the management of the Individualized Plan of Care for each participant.
  • Coordinate service provision based on participant acuity levels.
  • Support adherence to treatment recommendations and monitor participants' needs across various domains.
  • Meet client contact requirements, ensuring consistent engagement.
  • Maintain accurate documentation using designated care management portals and electronic health records.
  • Advocate for clients within the agency and with external service providers.
  • Promote wellness by connecting participants with relevant resources and services.
  • Educate participants and caregivers on managing chronic conditions and preventive care.
  • Assist clients in accessing necessary public benefits for healthcare services.
  • Facilitate effective communication with clients and their families, considering language and cultural preferences.
  • Participate in interdisciplinary care planning meetings to ensure comprehensive care delivery.
  • Identify community resources and manage referrals and follow-ups.
  • Engage in discharge planning for hospitalized clients to ensure continuity of care.
  • Participate in ongoing training to enhance skills relevant to the Care Coordinator role.
  • Ensure periodic evaluations and follow-ups for dental, vision, and hearing care.
  • Perform additional duties as required.

Qualifications:
Applicants should meet one or more of the following educational and credential requirements based on the acuity level of the children served:

  • Bachelor's degree with two years of relevant experience
  • Registered Nurse or Licensed Practical Nurse with two years of experience
  • Master's degree in a related field (licensure preferred)
  • Bilingual candidates are encouraged to apply

Experience:
Candidates should possess:

  • Experience serving children and families in various systems, including child welfare and mental health.
  • Knowledge of the needs of high-acuity children and families, supported by relevant experience or training.
  • Extensive experience in coordinating care for medically fragile children.
  • Experience in service coordination and community resource referrals.
