Position Overview

Under the supervision of the Director of Pharmacy Services, the Licensed Pharmacy Practitioner delivers indirect patient care. The

Clinical Pharmacy Specialist collaborates with nursing staff, colleagues, physicians, and other healthcare professionals to ensure

effective pharmaceutical services for patients. The Licensed Pharmacy Practitioner evaluates medication orders against the patient's

medication profile in alignment with the hospital's and pharmacy's philosophy, policies, procedures, and standards.

The Licensed Pharmacy Practitioner exhibits leadership and management capabilities while demonstrating a commitment to customer service, professional accountability, and personal growth.

Key Responsibilities Include:

1. Adhering to established Performance Standards, Policies, and Professional Behaviors.

2. Implementing all facets of the Pharmacy process to address the age-specific needs of patients, focusing on medication education, safety, and patient rights.

3. Monitoring Infection Control practices, participating in Performance Improvement and Risk Management initiatives, and demonstrating competency by fulfilling all established standards.

4. Engaging in a systematic decision-making process that includes recognition, data collection, prioritization, resource utilization, and collaborative suggestions with the healthcare team.

5. Maintaining productivity through punctuality, adhering to break time limits, and utilizing low-activity periods for the benefit of the Pharmacy and patients.

6. Demonstrating commitment through attendance at in-service training, involvement in projects and Performance Improvement, and pursuing personal development while maintaining current licensing requirements.

7. Taking responsibility for meeting competency standards and welcoming constructive feedback to implement necessary improvements.

8. Responding professionally to change, assisting colleagues, and fostering effective communication within the healthcare team.

9. Being adaptable in staffing and assignment adjustments.

10. Upholding a positive demeanor and effectively resolving conflicts.

11. Practicing cost containment strategies.

12. Complying with all policies and procedures, including dress code, code of conduct, customer service standards, and mandatory training.

13. Supervising Pharmacy Assistants.

14. Performing other related duties as assigned.

Education & Experience Requirements

Graduation from an accredited Pharmacy program with a minimum of a Bachelor of Science in Pharmacy.

Current registration in New York.

Successful completion of the Hospital and Departmental Orientation Program, in accordance with hospital and Pharmacy policies.

Ability to establish two-way communication by clearly defining expectations, utilizing various communication methods, actively listening to concerns, and responding appropriately.

Proficiency in pharmaceutical calculations.

Maintaining departmental competencies and required continuing education credits for licensure.

Physical/Mental Demands/Requirements & Work Environment

Exposure to chemicals necessary for task performance. Any hazardous chemicals are documented in the hospital's Safety Data Sheet (SDS) database, accessible through the hospital's intranet.

Ability to act professionally and serve as a role model.

Effective communication in written and verbal English.

Position requires the ability to ambulate, sit/stand for extended periods, and perform tasks requiring fine motor skills and hand-eye coordination.

Must meet all regulatory requirements and be able to concentrate on detailed tasks with interruptions.

Ability to understand and relate to multiple concepts and remember various tasks throughout the day.

Visual acuity for close and distant objects and the ability to hear normal sounds amidst background noise.

Tasks may involve exposure to chemotherapeutic agents and require knowledge of Universal Precaution techniques.

Must be physically capable of remaining stationary or ambulating for extended periods.

Ability to perform computer work and use transcription equipment for prolonged durations.

Coordination and dexterity required for operating equipment and handling materials.

Must be able to lift and transport heavy objects exceeding 30 lbs.

Ability to maintain focus for extended periods, even with distractions.

Performing repetitive tasks and effectively communicating with internal and external customers while inspecting deliveries for accuracy.
