Role Summary
The Licensed Pharmacy Practitioner operates under the guidance of the Pharmacy Services Director, delivering indirect patient care.
This role involves collaboration with nursing staff, fellow pharmacists, physicians, and other healthcare professionals to ensure effective pharmaceutical services for patients.
The practitioner meticulously reviews medication orders against patient profiles, adhering to established hospital and pharmacy protocols.
Key Responsibilities Include:
1. Complying with Performance Standards, Policies, and Behavioral Expectations.
2. Implementing comprehensive medication education and safety measures tailored to the specific needs of patients.
3. Engaging in Infection Control practices and participating in Performance Improvement and Risk Management initiatives.
4. Employing a structured decision-making process that includes recognition, data collection, and collaboration with the healthcare team.
5. Demonstrating punctuality and effective time management to optimize pharmacy operations.
6. Actively participating in professional development and maintaining current licensure requirements.
7. Accepting accountability for meeting competency standards and responding constructively to feedback.
8. Maintaining professionalism and effective communication within the healthcare team.
9. Adapting to staffing needs and assignment changes as required.
10. Upholding a positive demeanor and effectively resolving conflicts.
11. Practicing cost-effective measures in pharmacy operations.
12. Adhering to all relevant policies, dress codes, and attendance standards.
13. Supervising Pharmacy Assistants as needed.
14. Performing additional duties as assigned.
Qualifications:
Graduation from an accredited School of Pharmacy with a minimum of a Bachelor of Science in Pharmacy.
Current registration in New York.
Successful completion of the hospital's orientation program in accordance with pharmacy policies.

Ability to establish effective communication, actively listen, and respond appropriately.
Proficiency in pharmaceutical calculations and maintaining departmental competencies.
Commitment to professional development and maintaining required continuing education credits.
Ability to remain composed under pressure.
Physical and Mental Requirements:
Exposure to necessary chemicals as outlined in the hospital's Safety Data Sheet database.
Ability to perform tasks requiring fine motor skills and prolonged periods of standing or sitting.
Capability to concentrate on detailed tasks with minimal interruption.
Ability to communicate effectively in both written and verbal English.
Physical capability to transport objects weighing over 30 lbs.
Ability to manage repetitive tasks and maintain focus amidst distractions.
