Clinical Pharmacy Specialist collaborates with nursing staff, fellow pharmacists, physicians, and other healthcare professionals to ensure the effective delivery of
medication services to patients. This role involves a thorough review of medication orders against the patient's medication profile, adhering to the standards and protocols of the organization.
Key Responsibilities Include:
1. Adhering to established Performance Standards, Policies, and Professional Behaviors.
2. Implementing all aspects of the Pharmacy process to cater to the specific age-related needs of patients, focusing on medication education, safety, and patient rights.
3. Monitoring and applying Infection Control measures, participating in Performance Improvement and Risk Management initiatives, and demonstrating competency through adherence to established standards.
4. Engaging in a structured decision-making process that includes recognition, data collection, prioritization, resource utilization, and collaborative suggestions with the healthcare team.
5. Demonstrating productivity through punctuality, managing break times effectively, and utilizing low-activity periods for the benefit of the Pharmacy and patients.
6. Showing commitment through attendance at educational sessions, involvement in projects, and personal development while maintaining required licensure.
7. Taking responsibility for meeting competency standards and responding constructively to feedback.
8. Maintaining professionalism and cooperation in response to changes, assisting colleagues, and fostering effective communication within the healthcare team.
9. Being adaptable in staffing and assignments.
10. Upholding a positive demeanor and effectively resolving conflicts.
11. Practicing cost-effective measures.
12. Complying with all organizational policies, dress codes, codes of conduct, and mandatory training requirements.
13. Supervising Pharmacy Assistants.

Education & Experience Requirements
Graduation from an accredited Pharmacy program with a minimum of a Bachelor of Science in Pharmacy.
Current registration in the relevant jurisdiction.
Successful completion of the Hospital and Departmental Orientation Program, in line with organizational policies.

Ability to establish effective two-way communication by clearly defining expectations, utilizing various communication methods, actively listening to concerns, and responding appropriately.
Proficiency in pharmaceutical calculations.
Maintaining departmental competencies and required continuing education credits for licensure.

Ability to remain composed under pressure.
Physical/Mental Demands & Work Environment
Exposure to necessary chemicals for task performance, with all hazardous materials listed in the organization's Safety Data Sheet database.
Ability to communicate effectively in written and verbal English.
Position requires the ability to ambulate, sit or stand for extended periods, and perform tasks requiring manual dexterity and coordination.
Must be capable of concentrating on detailed tasks for extended periods, even amidst interruptions.
Ability to perceive objects at varying distances and hear normal sounds amidst background noise.
Tasks may involve exposure to chemotherapeutic agents, necessitating knowledge of Universal Precaution techniques.
Physical capability to remain stationary or mobile for prolonged periods and perform computer-related tasks.
Must be able to lift and transport heavy items exceeding 30 lbs.
Ability to focus for extended periods and perform repetitive functions.
Must effectively communicate with both internal and external customers and verify the accuracy of deliveries.
