Key Responsibilities- Assess and provide care to a selected group of patients in collaboration with physicians.
- Carry out the plan of care for patients, working closely with physicians and other clinicians in a team approach to patient care.
- Perform health assessments, including patient history and physical examination.
- Order diagnostic tests, evaluate special test and study results, and incorporate them into the plan of care.
- Formulate appropriate diagnoses based on history, physical examination, and clinical findings.
- Plan and provide care within the framework of policies, procedures, privileges, and available resources, applying relevant research.
- Involve patients and families in planning care.
- Collaborate with physicians and other healthcare providers to coordinate care and evaluate the plan of care and patient progress.
- Prescribe pharmacologic and non-pharmacologic therapies, according to privileges.
- Order, perform, and interpret diagnostic procedures and tests within the scope of privileging.
- Provide patient and family education related to health status.
- Evaluate the effectiveness of the plan of care and interventions by comparing patient responses to planned outcomes.
- Collaborate and communicate effectively with all members of the healthcare team.
- Document in a thorough and timely manner.
- Master's Degree in Nursing from an accredited school.
- Current NYS license to practice as a Registered Professional Nurse.
- Current NYS license as a Nurse Practitioner.
- National Certification as a Nurse Practitioner.
- Maintains a current collaborative practice agreement.
- BCLS certification required; ACLS certification preferred.
- Minimum two years' experience in an acute care setting preferred.
