We are currently seeking Full Time and Per Diem Licensed Vocational Nurses (LVNs) to deliver continuous care for hospice patients in their residences.
Applicants must be prepared to travel to various locations.
Available Shifts: 4pm-12am & 12am-8am
Hourly Rate: $ plus mileage (based on experience)

POSITION OVERVIEW
We are in search of a qualified California licensed vocational nurse (LVN) to take on on-call and/or continuous care shifts. The LVN operates under the supervision of a Registered Nurse, providing nursing and palliative care to designated hospice patients. The On-Call / Continuous Care Hospice LVN is responsible for delivering intermittent visits and/or shifts of skilled nursing care, administering prescribed medical treatments, including pain management and symptom control, conducting nursing assessments, and educating patients, families, or primary caregivers about the disease trajectory, care techniques, and other health-related measures. The LVN will also collaborate in the development and execution of care plans with other members of the hospice interdisciplinary team. Proficiency in computer usage and comfort with electronic medical records (EMR) is essential. A compassionate approach to caring for hospice patients and families, along with effective communication skills for interaction with the hospice team, is required. On-call and continuous care shifts often occur during evenings and weekends.
QUALIFICATIONS:
- Graduate of an accredited vocational/practical nursing program.
- Current LVN license in the State.
- Certification in Hospice and Palliative Vocational/Practical Nursing is preferred.
- Minimum of one year of experience as an LVN/LPN.
- Valid driver's license, auto insurance, and reliable transportation.
