Role: Licensed Vocational Nurse - Hospice
Bayview Hospice Care is seeking dedicated LVNs to provide home hospice services in various regions. Our organization prioritizes exceptional outcomes for our clients, delivering personalized customer service and continuous clinical education for our staff. We uphold dignity and respect for all patients, families, physicians, and team members.
Key Responsibilities:
- Deliver professional nursing care to patients, families, and caregivers.
- Manage and provide comprehensive care, including clinical, palliative, and symptom management, while assessing the physical, psychological, social, and spiritual needs of assigned patients.
- Implement and evaluate care plans, ensuring interventions meet the needs of patients and families.
- Collaborate with interdisciplinary team members to address all patient and family care requirements.
- Participate in the planning, execution, and assessment of medical and nursing care alongside the Interdisciplinary Team.
Qualifications:
- 1-2 years of experience in home health or hospice care is preferred.
- Strong clinical and communication skills, with a passion for working with the elderly.
- Valid and unrestricted Licensed Vocational Nurse license in California.
- Ability to function effectively within an interdisciplinary team environment.
COVID-19 Safety Measures:
- Adherence to social distancing guidelines.
- Implementation of sanitizing and cleaning protocols.
Personal Protective Equipment will be provided.
