About the Role

The Clinical Care Coordinator is a key member of our interdisciplinary team at Roze Room Hospice, responsible for providing exceptional patient care and support to individuals and families in need of hospice and palliative services.

Key Responsibilities
  • Engage with patients and families to educate them about hospice and palliative care services, and assist with the election of hospice benefit.
  • Coordinate patient discharge from hospital into hospice or palliative care programs, and perform start of care and initial nursing assessment.
  • Work cooperatively with the hospice and/or palliative care teams to ensure seamless care coordination.
  • Utilize strong communication and interpersonal skills to build relationships with patients, families, and healthcare professionals.
  • Meet with patients and families in person and/or remotely to provide education and support.
Requirements
  • Graduate of an accredited school of nursing with a current nursing license in California.
  • Certification in Hospice and Palliative Nursing is highly encouraged.
  • At least one year of experience as a professional nurse within the last three years.
  • Excellent verbal and written communication skills.
  • Spanish speaking is a plus, but not required.
  • Valid driver's license, and auto insurance, reliable and well-maintained transportation.
