Key Responsibilities:
The Registered Nurse will be responsible for:
- Initiating and coordinating individualized hospice care plans.
- Supervising Certified Home Health Aides (CHHAs) and Licensed Vocational Nurses (LVNs).
- Delivering skilled nursing care and palliative services to patients in their residences.
- Collaborating with the attending physician, Hospice Medical Director, and the Interdisciplinary Hospice Team.
- Regularly reviewing and updating care plans, performing medical treatments, and managing pain and symptoms.
- Conducting assessments and evaluations, while providing education and support to patients and their caregivers.
- Utilizing electronic medical records efficiently.
Qualifications:
To be considered for this position, candidates must possess:
- A degree from an accredited nursing program.
- A valid California RN license; a BSN is preferred. Certification in Hospice and Palliative Nursing is encouraged.
- A minimum of one year of professional nursing experience within the last three years, or a Baccalaureate in Nursing from an accredited program.
- A valid driver's license, auto insurance, and reliable transportation.
- Spanish language skills are a plus, but not mandatory.
