The Licensed Practical/Vocational Nurse plays a crucial role in delivering direct patient care under the guidance of a registered nurse. Key responsibilities include adhering to the care plan, administering treatments, and collaborating with team members to achieve optimal patient outcomes.

COMPENSATION AND BENEFITS:

PAY STRUCTURE AND PERKS

Wage: $45 per visit

Mileage reimbursement based on patient distance and caseload

Paid sick leave available per state regulations

Job Type: per Diem (opportunities for growth and additional benefits for full-time employees)

Spanish-speaking candidates are preferred.

KEY RESPONSIBILITIES:

ESSENTIAL DUTIES:

  • Deliver direct patient care as outlined in the State Nurse Practice Act.
  • Implement current nursing practices following patient assessments and approved care plans initiated by the registered nurse.
  • Maintain accurate and timely documentation of patient services in accordance with the care plan.
  • Educate patients and their families/caregivers about diagnoses and self-care plans.
  • Coordinate hospice services, reporting identified needs to interdisciplinary team members, including hospice aides, occupational therapists, and clinical supervisors.
  • Utilize equipment and supplies efficiently and effectively.
  • Provide appropriate pain and symptom management, evaluating patient responses to treatments and medications.
  • Engage in personal and professional development, contributing to the organization's quality management initiatives.
  • Perform additional duties as assigned by the registered nurse.

QUALIFICATIONS & REQUIREMENTS Successful candidates must meet the following criteria:
  • Graduate of an accredited practical nurse or vocational nursing program.
  • Three years of nursing experience, preferably in community health, hospice, or medical/surgical settings.
  • Current licensure as an LPN/LVN in the state.
  • Adherence to accepted professional standards and practices.
  • Strong verbal and written communication skills, along with excellent organizational abilities.
  • Must possess a valid driver's license and have access to a reliable, insured vehicle.
  • Understanding of hospice philosophy and the needs of terminally ill patients.
