Facility Overview:

  • Our facilities specialize in Intermediate Care for individuals with disabilities (ICF/DD-H and ICF/DD-N)
  • Caregiver (DSP) to residents ratio typically stands at 1:3, occasionally reaching 1:1 support
  • Nursing responsibilities generally involve a caseload of 6-15 residents
  • Our homes are designed to be intimate, featuring 6-bed and 15-bed configurations
  • High-acuity care is restricted or not permitted by facility regulations
  • Our Intermediate Care Facilities administer wound care as per licensing standards, addressing simple wounds up to stage I pressure ulcers
  • Hospice services are limited and provided in collaboration with professional hospice care providers

As a Licensed Vocational Nurse (LVN), you will play a crucial role in our team, delivering high-quality nursing care and support to individuals with disabilities within our Intermediate Care Facilities (ICF). This position allows you to work in a small, cohesive community where you can cultivate meaningful relationships with both the residents and your dedicated colleagues.

Key Responsibilities of the Licensed Vocational Nurse (LVN):

  • Deliver skilled nursing care to individuals with disabilities in an Intermediate Care Facility environment
  • Implement and supervise healthcare plans for residents
  • Administer medications and perform necessary medical procedures
  • Collaborate with a multi-disciplinary team to ensure residents' well-being
  • Maintain precise medical records and documentation
  • Provide support and guidance to Direct Support Professionals (DSP)
  • Engage in complex case management and pursue continuous learning opportunities

Qualifications for the Licensed Vocational Nurse (LVN):

  • Valid Licensed Vocational Nurse (LVN) license in California
  • CPR certification
  • Strong communication and interpersonal abilities
  • A genuine commitment to providing quality care and support to individuals with disabilities
  • Ability to thrive in a team-oriented setting
  • Willingness to adapt to the evolving needs of residents and the facility
