The Licensed Practical/Vocational Nurse plays a crucial role in delivering direct patient care under the guidance of a registered nurse. Key responsibilities include adhering to the care plan, administering treatments, and collaborating with team members to achieve optimal patient outcomes.

COMPENSATION AND BENEFITS:

PAY AND BENEFITS

Wage: $45 per visit

Mileage reimbursement based on patient distance and caseload

Paid sick leave available per California regulations

Job Type: per Diem (opportunities for growth and additional benefits for full-time employees)

Spanish-speaking candidates are preferred.

KEY RESPONSIBILITIES:

ESSENTIAL DUTIES:

  • Deliver direct patient care as outlined in the State Nurse Practice Act.
  • Implement current nursing practices following patient assessments and approved care plans initiated by the registered nurse.
  • Maintain accurate and timely documentation of patient services to reflect the care plan.
  • Provide education and information to patients and families regarding diagnoses and self-care plans.
  • Coordinate hospice services and report identified needs to interdisciplinary team members, including hospice aides, occupational therapists, physical therapists, social workers, speech therapists, dietitians, or clinical supervisors.
  • Utilize equipment and supplies effectively and efficiently.
  • Administer appropriate pain and symptom management, evaluating patient responses to treatments and medications.
  • Engage in personal and professional growth and development, participating in the organization's quality management program.
  • Perform additional duties as assigned by the registered nurse.

QUALIFICATIONS & REQUIREMENTS Successful candidates must meet the following criteria:
  • Graduate of an accredited practical nurse or vocational nursing program.
  • Three years of nursing experience, preferably in community health, hospice, or medical/surgical settings.
  • Current licensure as an LPN/LVN in the State.
  • Adhere to accepted professional standards and practices.
  • Exhibit strong verbal and written communication and organizational skills.
  • Possess a valid driver's license and an insured automobile in good working condition.
  • Understand the philosophy of hospice care and the needs of terminally ill patients.
