The hospice aide serves as a vital member of the interdisciplinary team, working under the guidance of a registered nurse to provide essential services that cater to the personal needs of patients, ensuring their comfort and dignity.
KEY RESPONSIBILITIES
The responsibilities of the hospice aide encompass, but are not limited to, the following:
1. Delivering personal care services as per patient assignments and responding promptly to their requests. If a task cannot be performed, it is imperative to inform the Case Manager without delay.
2. Ensuring the safety of the patient by utilizing equipment correctly (e.g., foot stools, side rails, oxygen equipment).
3. Providing personal care, which includes bathing, back rubs, oral hygiene, hair washing, and changing bed linens as required.
4. Assisting patients with dressing and undressing as needed.
5. Planning and preparing nutritious meals, including grocery shopping, as directed.
6. Aiding in feeding patients as assigned.
7. Monitoring and caring for the patient's skin to prevent any breakdown of tissue over bony areas.
8. Reporting any changes in the patient's condition to the Case Manager, while also being aware of interpersonal dynamics involving caregivers or family members.
9. Supporting patients in ambulation and exercises as instructed by the hospice nurse or therapist.
10. Assisting with bedpans and urinals, and providing help with light laundry tasks as assigned.
11. Performing range of motion exercises and other simple procedures as an extension of therapy services, ensuring all skills are demonstrated competently.
13. Maintaining a clean and orderly living environment for the patient, as directed.
14. Adhering to the organization's documentation standards and maintaining professional conduct at all times.
15. Participating in quality assessment and performance improvement initiatives.
16. Demonstrating respect for the patient and their family/caregiver environment, as well as the personal needs of the patient.
It is important to note that the hospice aide does not engage in any activities classified as nursing practices according to the Nurse Practice Act.
The hospice aide is not authorized to administer medications, accept physician orders, or perform procedures that require the expertise of a licensed nurse, such as sterile techniques.
This job description provides a general overview of the primary duties and responsibilities associated with this position. The employee may be asked to perform additional job-related tasks as necessary.
QUALIFICATIONS (Education, Experience, Knowledge, Skills & Abilities)
1. Completion of training requirements as mandated by the State.
2. Minimum age of 18 years.
3. Ability to read, comprehend, and follow written instructions, as well as document care provided.
4. Understanding of hospice philosophy and comfort in providing specialized care to individuals with terminal illnesses.
5. Positive references from previous employers.
6. Self-motivated with the capability to work independently with minimal supervision.
7. Comfort with issues surrounding death and dying, exhibiting a calm demeanor in patient homes.
8. Empathy for the needs of the ill, injured, frail, and impaired individuals.
9. Current CPR certification is required.
10. Demonstrates patience, tact, and maintains good personal hygiene.
11. Valid driver's license with an insured vehicle that meets organizational standards and is in good working condition.
12. Strong oral and written communication skills.
13. Effective organizational abilities.
