Position Overview: The Case Manager is essential in assisting individuals facing homelessness. This role encompasses guiding clients through various phases of housing instability, including outreach, temporary accommodations, and long-term housing solutions. The Case Manager will deliver tailored support to ensure clients can access and sustain vital services such as healthcare, social support, and community resources.



Key Responsibilities:

  • Client Support: Guide clients through all stages of homelessness, from outreach to permanent housing. Provide customized assistance to help clients identify their strengths, challenges, health requirements, and housing aspirations.
  • Service Coordination: Aid clients in navigating healthcare and social service systems. Develop and execute a comprehensive care strategy that encompasses housing and health objectives.
  • Assessment & Planning: Perform intake evaluations and formulate Individualized Service Plans (ISP) that address housing goals, income stability, and other personal aims such as medical, mental health, substance use, financial resources, vocational, and social support needs.
  • Resource Connection: Offer referrals and link clients to resources that align with their ISP objectives. Conduct program-specific assessments and perform regular reviews to monitor progress and modify plans as necessary.
  • Documentation & Compliance: Ensure accurate and timely case file documentation and client records in line with program and organizational standards. Maintain all required documents to be current and complete.
  • Collaboration & Teamwork: Collaborate effectively with colleagues, clients, and stakeholders to achieve collective goals. Cultivate positive working relationships and acknowledge the contributions of others.
  • Communication: Effectively communicate information to individuals and groups, both verbally and in writing, to promote understanding and engagement.
  • Critical Thinking & Problem Solving: Evaluate information critically to make informed decisions that lead to desired outcomes. Address challenges with logical reasoning and sound judgment.
  • Ethics & Boundaries: Adhere to ethical and professional standards, demonstrating integrity while maintaining professional boundaries.
  • Driving: Travel as necessary to meet clients and access resources is a fundamental aspect of this role.

Qualifications:

  • Demonstrated experience in case management or a related discipline.
  • Strong knowledge of homelessness and the services needed to address it.
  • Excellent communication and interpersonal abilities.
  • Capacity to work collaboratively within a team framework.
  • Proficient in critical thinking and problem-solving.
  • Commitment to upholding ethical standards and professional boundaries.
  • Valid driver's license and dependable transportation.

Preferred Skills:

  • Familiarity with local resources and support systems in the area.
  • Experience with program-specific assessments and documentation processes.
