Probationary Period: 90 days

Role Summary: Under the guidance of the Manager of Housing Case Management, the Housing Stabilization Specialist I will play a crucial role in fulfilling contract obligations while delivering supportive services through a client-focused lens. The ideal candidate will demonstrate exceptional sensitivity to the complexities faced by individuals experiencing homelessness, along with various medical and behavioral health challenges as they transition into and sustain permanent supportive housing. The specialist will ensure compliance with local, state, and federal regulations and accreditation standards while providing quality care.

Key Responsibilities:

  1. Serve as the primary contact for high-need clients throughout their engagement in the program.
  2. Conduct comprehensive intake and Bio-Psychosocial assessments for new participants to identify needs, eligibility, interests, and goals.
  3. Implement necessary interventions to address barriers to housing.
  4. Connect clients to medical homes and supportive services, including mental health, health, substance abuse, employment, and education.
  5. Facilitate access to resources such as DPSS, Social Security, and Medi-Cal, acting as a liaison for clients.
  6. Assist clients in finding suitable housing options through field-based searches.
  7. Support clients in completing housing applications and securing financial assistance.
  8. Collaborate with clients and their support systems to develop individual care plans aimed at achieving educational, employment, housing stability, and health goals.
  9. Conduct regular assessments and maintain documentation of client progress.
  10. Perform home visits to ensure clients are successfully maintaining their housing.
  11. Work collaboratively within an interdisciplinary team to meet the needs of high-need clients.
  12. Participate in case conferences to coordinate services with internal and external providers.
  13. Maintain accurate client data and documentation in compliance with contract requirements.
  14. Engage with clients and landlords during flexible hours, including evenings and weekends.
  15. Demonstrate professionalism in all interactions with staff and clients.
  16. Adhere to OSHA, HIPAA, and other relevant regulations.
  17. Other duties as assigned.

Qualifications:

  1. Minimum of three years of experience in direct mental health or intensive case management services for individuals experiencing homelessness.
  2. Bachelor's degree required, preferably in social work or a related field.
  3. Understanding of homeless populations and related systems, programs, and federal policies.
  4. Valid driver's license and reliable transportation.
  5. Training in Motivational Interviewing and Trauma-Informed Care is a plus.
  6. Strong communication skills, both verbal and written, with attention to detail.
  7. Proficient in Microsoft Office and electronic health record systems.
  8. Ability to work independently and manage multiple tasks effectively.
  9. Bilingual skills (English/Spanish) are advantageous.

Training and Certification:

  1. Bachelor's Degree required.
  2. AHA CPR certification.
