Compensation: $24-27 Per Hour
As the primary organization for the SPA 6 Coordinated Entry System (CES) catering to Single Adults, HOPICS plays a crucial role in identifying the most at-risk homeless individuals and matching them to housing subsidies through a prioritization framework established by the Continuum of Care. Collaborating with CES-funded partners within SPA 6, the CES Time Limited Subsidy Matcher will efficiently coordinate and monitor housing program placements while identifying areas for data and system enhancement within the CES.
Key Responsibilities- Articulate the HUD definition of homelessness and chronic homelessness accurately.
- Communicate the services offered through the SPA 6 CES for Single Adults.
- Understand and explain various housing resources, subsidies, and eligibility criteria.
- Familiarize with the LAHSA CES Matching Policy.
- Oversee time-limited subsidy matches and facilitate data sharing among CES subcontractors and partner agencies.
- Promote collaboration among interim housing, housing navigation, and time-limited subsidy providers to enhance referral opportunities and expedite placements.
- Provide ongoing support and conduct training sessions for new staff regarding the Time Limited Subsidy Matching process.
- Participate in Time Limited Subsidy-specific meetings and trainings.
- Engage in weekly case conferencing meetings in SPA 6 to ensure accurate matching to available time-limited subsidy resources.
- Coordinate services, training, and dissemination of CES matching information for SPA 6.
- Advocate for system coordination by collaborating with regional leads and partner agencies to identify process barriers and develop solutions.
- Facilitate case consultation meetings with community providers to enhance collaborative efforts.
- Provide technical assistance on HMIS linkages and the referral process for the flow from Interim Housing to Housing Navigation to Time Limited Subsidies.
- Maintain precise documentation of matches in HMIS and related trackers.
- Track and report the number of homeless Time Limited Subsidy slots weekly.
- Manage the Community Queue system registry in HMIS to monitor the allocation of Time Limited Subsidy slots within SPA 6.
- Maintain effective communication logs to illustrate the significance of collaboration through outcomes and achievements.
- Participate in CES infrastructure development within the organization and community.
- Stay informed about eligibility criteria and documentation standards for all housing subsidy programs.
- Contribute to achieving CES contractual performance targets and division-wide goals.
- Adhere to the organization's Code of Ethics and Core Values.
Minimum Requirements:
- High School diploma with 3 years of experience in homeless services or a related field, or a bachelor’s degree with 1 year of experience in homeless services or a related field.
- Familiarity with short and long-term permanent housing subsidies in LA County.
- Understanding of Homeless Service Providers and the Coordinated Entry System.
- Strong communication skills with the ability to engage with diverse stakeholders in a culturally competent manner.
- Excellent organizational, time management, and interpersonal skills.
Preferred Qualifications:
- Proficiency in Microsoft Office, Excel, HMIS, and other database programs.
- Background in community-based treatment providers.
The role involves both office and fieldwork, requiring adaptability to a dynamic environment with multiple priorities. The incumbent must be able to engage with clients in various settings, including shelters, and manage interactions with individuals displaying challenging behaviors.
Physical and Mental RequirementsThe position necessitates the ability to perform tasks such as sitting, standing, walking, and carrying up to 20 lbs. The incumbent must also be capable of handling unpredictable changes in priorities and effectively managing interpersonal relationships.
