Position Overview:

The role of the Case Manager involves overseeing client care through comprehensive assessment of treatment needs, development, monitoring, and evaluation of treatment plans, and facilitating collaborative approaches among various disciplines.

Key Responsibilities:

  • Client Admission: Review client records and applications, and conduct orientations to ensure a smooth onboarding process.
  • Intake Assessment: Conduct intake interviews to identify client needs for therapeutic medical, psycho-social, and psychiatric evaluations.
  • Treatment Program Development: Establish and coordinate treatment schedules and routines, ensuring all necessary resources, including transportation, are arranged.
  • Case Monitoring: Verify client attendance, observe treatment responses, advocate for necessary services, and provide personal support during crises.
  • Record Maintenance: Keep accurate and up-to-date client records by reviewing case notes and logging significant events and progress.
  • Progress Communication: Conduct weekly interdisciplinary meetings to discuss client progress and challenges, ensuring all team members are informed.
  • Discharge Planning: Prepare clients for discharge by reviewing and enhancing discharge plans, coordinating post-discharge requirements, and training family members.
  • Staff Development: Enhance staff skills by providing educational resources and evaluating the application of new knowledge to improve treatment outcomes.
  • Process Improvement: Continuously evaluate and redesign processes to enhance treatment results and implement necessary changes.
  • Budget Management: Monitor expenses and implement cost-saving measures to meet budgetary requirements.
  • Reporting: Prepare comprehensive reports by collecting and analyzing treatment data, summarizing results, and compiling statistics.
  • Professional Development: Stay updated with industry knowledge through educational opportunities and professional networking.
  • Organizational Contribution: Contribute to the reputation of the department and organization by taking ownership of new initiatives and exploring ways to add value.

Required Skills and Qualifications:

  • Strong documentation and analytical skills

  • Proficient in conflict resolution and people management
  • High level of integrity

Education and Experience:

A Master's degree in social work is preferred, along with a minimum of one year of experience in a Rapid Rehousing program. Experience supporting individuals with disabilities, mental health challenges, or behavioral issues is also essential.
