The Registered Nurse Care Coordinator plays a vital role in ensuring seamless patient care from admission to discharge. This position involves:

  • Coordinating patient care to facilitate smooth transitions and high-quality outcomes.
  • Engaging with patients and families to address care needs and barriers.
  • Collaborating with interdisciplinary teams to develop and implement effective discharge plans.
  • Identifying and recommending post-acute services and completing necessary referrals.

The RN Care Coordinator is an essential member of the care team, participating in rounds and care conferences to advocate for patients and families. You will work closely with the hospital team to ensure that patient preferences and needs are met while aligning with care coordination requirements.

Minimum Qualifications:
To be considered for this role, candidates must possess a valid and active nursing license. A Bachelor’s degree in nursing is preferred, along with at least two years of recent healthcare experience. Successful candidates will meet quality and productivity expectations and complete annual competencies.
