Client Care Coordination
- Ensures client well-being by evaluating treatment requirements; crafting, overseeing, and assessing care plans and progress; promoting collaborative approaches; supervising staff performance.
- Onboards new clients by reviewing documentation and applications, conducting introductory sessions.
- Identifies client needs through comprehensive intake interviews; assessing the necessity for therapeutic medical, psycho-social, and psychiatric evaluations; reviewing therapist assessments, treatment goals, and strategies.
- Establishes care programs by scheduling and organizing routines; coordinating services offered; arranging necessary resources, including transportation and support.
- Monitors client cases by confirming attendance; observing and assessing treatments and responses; advocating for essential services and entitlements; sourcing additional resources; intervening during crises; providing personal assistance.
- Maintains client records by reviewing case notes, documenting events and progress.
- Communicates client progress by conducting weekly interdisciplinary meetings and evaluations; sharing results and challenges with the therapeutic team and family; identifying treatment influences.
- Facilitates client discharge by reviewing and enhancing discharge plans; coordinating discharge and post-discharge needs; training family members; providing necessary resources.
- Enhances staff capabilities by offering educational resources; balancing work demands with learning opportunities; assessing the application of learning to treatment outcomes.
- Improves treatment outcomes by analyzing, evaluating, and redesigning processes; implementing changes; revising policies and procedures.
- Adheres to budgetary constraints by monitoring expenditures, implementing cost-saving measures.
- Prepares reports by gathering, analyzing, and summarizing treatment data and outcomes; compiling statistics; completing funding applications.
- Enhances professional knowledge by engaging in educational opportunities; reading industry publications; maintaining professional networks; participating in relevant organizations.
- Boosts department and organizational reputation by taking ownership of new and diverse requests; exploring opportunities to add value to job accomplishments.
