GENERAL OVERVIEW:

The Speech-Language Pathologist operates as an autonomous healthcare provider dedicated to delivering tailored patient care. This role involves the assessment and treatment of various disorders related to speech, language, social communication, cognitive communication, voice, fluency, and swallowing in both pediatric and adult populations upon physician referral. The Speech-Language Pathologist is tasked with evaluating referred patients, setting patient and family-centered objectives, and formulating and implementing treatment strategies aimed at enhancing or maximizing independence in the post-discharge environment.

KEY RESPONSIBILITIES:

+ Conduct initial assessments and follow-up evaluations concerning the patient's status in relation to speech, language, social and cognitive communication, voice, fluency, and/or swallowing, in alignment with physician directives.

+ Execute evaluations that include a comprehensive review of the patient's medical, surgical, and social history. This involves interpreting both formal and informal assessments across various domains: Motor Speech, Verbal Expression, Written Expression, Voice and Resonance, Auditory Comprehension, Reading Comprehension, Hearing impacts on speech and language, Swallowing function, Non-Verbal Communication, Orientation, Attention, Problem Solving and Reasoning, Memory, Oral Motor Function, Pragmatics, and Visual Perceptual Skills. Objective testing may be intricate and require adjustments based on medical comorbidities.

+ Develop personalized care plans that align therapist and patient/family-directed goals to achieve realistic outcomes for prevention, intervention, and sustainable wellness throughout the patient's life.

+ Implement and advance evidence-based therapeutic interventions in accordance with established and revised goals and care plans, adapting to the patient's response to treatment. Supervise support staff as necessary and appropriate.

+ Formulate and coordinate a comprehensive discharge plan that includes recommendations tailored to the patient's needs in collaboration with other healthcare professionals.

+ Engage in peer consultations regarding patient care, which is encouraged and supported. More experienced and specialized therapists are expected to provide formal or informal mentoring.

+ Maintain educational standards by participating in and providing in-service training and attending staff meetings regularly. Responsibilities also include assisting with the orientation of new therapy staff, serving as a Clinical Instructor for students, acting as a clinical fellowship supervisor, and mentoring colleagues.

+ Evaluate patients' educational needs, address any barriers to understanding or learning, and involve the patient, family, and/or caregiver in the therapeutic process according to the patient's preferences. Collaborate and communicate with other healthcare team members to optimize therapy benefits and ensure quality care.

+ Complete necessary medical documentation, including initial evaluations, re-evaluations, daily notes, care plans with associated certification dates, physician correspondence, insurance authorizations, and discharge summaries, which substantiate skilled care in accordance with professional, departmental, and payer standards.

+ Fulfill administrative responsibilities related to patient care, which may encompass:

+ Scheduling assistance

+ Monitoring patient attendance

+ Ensuring accurate and timely billing practices

+ Verifying appropriate referral and diagnostic coding

+ Utilizing only medically necessary services and visit counts

+ Adhering to insurance authorizations and tracking limitations

+ Participating in peer reviews

+ Engaging in performance and process improvement initiatives

+ Contributing to program development

+ Involvement in community outreach

+ Other related activities

+ Support the financial sustainability of Corewell Health by meeting departmental productivity metrics.

+ Adhere to work schedule expectations based on site needs and operational hours required for patient care. This may necessitate working at various locations depending on patient volume or other factors. Inpatient scheduling requirements are based on a seven-day operational model, including availability for weekend and holiday rotations.

QUALIFICATIONS:

To successfully perform this role, an individual must be capable of fulfilling each essential duty effectively. The qualifications listed below represent the knowledge, skills, and/or abilities required. Reasonable accommodations may be made to enable individuals with disabilities to perform essential duties.

A. Education / Training:

Master's Degree, Certificate of Clinical Competence (ASHA) or Clinical Fellowship Year (CFY), ASHA.

B. Work Experience:

Experience in assessing and treating neurological speech and language disorders in both adults and children, including conditions such as aphasia, apraxia, dysarthria, dysphagia, laryngectomy, stuttering, and voice disorders. Familiarity with head and neck cancer, tracheostomies, cognitive impairments, TBI, dementia, and modified barium swallow studies is essential.

C. Certification, Licensure, Registration:

Certificate of Clinical Competence from the American Speech-Language-Hearing Association (required) or Clinical Fellowship Year (CFY) from the American Speech-Language-Hearing Association. Licensure in Speech Pathology in Michigan (required) or a temporary license during the CFY (required).
