Key Responsibilities as a Hospice RN/Case Manager

  • Report directly to the Administrator, Executive Director, or Patient Care Manager and maintain oversight and professional management of each patient.
  • Serve as the primary communication link between the physician, patient/family, and hospice team.
  • Deliver direct care services to hospice patients and offer supportive assistance to their families while fostering a trusting and compassionate relationship.
  • Empower caregivers by providing education on the care of their loved ones.
  • Collaborate with an interdisciplinary team, including Licensed Practical/Vocational Nurses (LPN or LVN), Certified Nursing Assistants (CNA), and support staff to ensure comfort and dignity.
  • Assess patient and family needs and assist in developing and implementing individualized care plans.
  • Maintain accurate clinical records for all assigned patients.
  • Engage in quality assessment and performance improvement initiatives.

Candidate Profile

Essential Qualifications for Hospice Registered Nurse:

  • A compassionate and empathetic approach to patient care.
  • Comfortable providing care in various home settings.
  • Ability to remain calm and flexible while supporting individuals in crisis.
  • Current RN license in the state of employment (or eligibility to obtain).
  • Minimum of 1 year of nursing experience in hospice, home health, ICU, geriatrics, or oncology preferred.
  • Additional preferred experience includes hospital Staff RN, Med Surg, PACU, Float pool, ER, critical care, acute care, cardiac, geriatrics, admissions, wound care, telemetry, or travel nursing.
  • Valid driver's license and automobile insurance.
  • Current CPR certification.
