Every day presents an opportunity to enhance the personal care experience and improve the quality of life for our patients. We provide tailored personal care, palliative services, and hospice support to ensure individuals receive the compassionate care they deserve.
Key Responsibilities as a Hospice RN/Case Manager
- Report directly to the Administrator, Executive Director, or Patient Care Manager and maintain oversight and professional management of each patient.
- Serve as the primary communication link between the physician, patient/family, and hospice team.
- Deliver direct care services to hospice patients and offer supportive assistance to their families while fostering a trusting and compassionate relationship.
- Empower caregivers by providing education on the care of their loved ones.
- Collaborate with an interdisciplinary team, including Licensed Practical/Vocational Nurses (LPN or LVN), Certified Nursing Assistants (CNA), and support staff to ensure comfort and dignity.
- Assess patient and family needs and assist in developing and implementing individualized care plans.
- Maintain accurate clinical records for all assigned patients.
- Engage in quality assessment and performance improvement initiatives.
Essential Qualifications for Hospice Registered Nurse:
- A compassionate and empathetic approach to patient care.
- Comfortable providing care in various home settings.
- Ability to remain calm and flexible while supporting individuals in crisis.
- Current RN license in the state of employment (or eligibility to obtain).
- Minimum of 1 year of nursing experience in hospice, home health, ICU, geriatrics, or oncology preferred.
- Additional preferred experience includes hospital Staff RN, Med Surg, PACU, Float pool, ER, critical care, acute care, cardiac, geriatrics, admissions, wound care, telemetry, or travel nursing.
- Valid driver's license and automobile insurance.
- Current CPR certification.
