Key Responsibilities as a Hospice RN/RN Case Manager
- Report directly to the Administrator, Executive Director, or Patient Care Manager Senior, maintaining primary oversight and professional management of each patient.
- Serve as the main point of contact between the physician, patient/family, and hospice team.
- Deliver direct care services to hospice patients while offering supportive care to their families, ensuring a compassionate and trusting relationship.
- Empower caregivers by providing education on caring for their loved ones.
- Collaborate with an interdisciplinary team, including Licensed Practical/Vocational Nurses (LPN or LVN), Certified Nursing Assistants (CNA), and support staff to ensure comfort, peace, and dignity.
- Assess patient and family needs, assisting in the development and implementation of personalized care plans.
- Maintain accurate clinical records for all assigned patients.
- Engage in company quality assessment and performance improvement initiatives.
Requirements for Hospice Registered Nurse:
- A compassionate heart: empathetic and kind towards patients and their families.
- Comfortable providing care in various settings.
- Exhibit flexibility and composure while supporting others during challenging times.
- Possess an RN license in the state of employment (or eligibility to obtain).
- 1+ years of nursing experience in hospice, home health, ICU, geriatrics, or oncology is strongly preferred.
- Additional preferred experience includes hospital Staff RN, Med Surg, PACU, Float pool, ER, critical care, acute care, cardiac, geriatrics, admissions, wound care, telemetry, or travel nursing.
- Valid driver's license and automobile insurance.
- Current CPR certification.
