Key Responsibilities
- Deliver personal care services as assigned, responding promptly to patient requests
- Communicate effectively with the Case Manager regarding any challenges in task completion
- Employ positive communication techniques with patients and families, ensuring confidentiality and respect
- Ensure patient safety through proper use of equipment and adherence to safety protocols
- Provide personal care including bathing, oral hygiene, and changing bed linens as required
- Assist patients with dressing and undressing as needed
- Prepare nutritious meals and assist with feeding as assigned
- Monitor and record vital signs as per care plans
- Observe and report any significant changes in patient condition to the Case Manager
- Support patients with mobility and exercise as directed by healthcare professionals
- Maintain cleanliness and organization of the patient's living space
- Follow organizational policies and procedures for documentation and care standards
- Participate in quality improvement initiatives as required

Qualifications
- Completion of state-mandated training requirements
- Minimum of 75 hours of classroom and supervised practical training
- At least six months of experience in providing personal care
- Must be at least 18 years old
- Ability to comprehend and follow written instructions
- Understanding of hospice care principles and comfort in working with terminally ill patients
- Strong references from previous employers
- Capability to work independently with minimal supervision
- Comfort with end-of-life issues and maintaining a calm demeanor in patient homes
- Current CPR certification required
- Excellent communication and organizational skills
