You are a registered nurse with experience in hospice or home health care, passionate about delivering exceptional comfort and care to patients in their residences, assisted living facilities, or skilled nursing environments. You possess excellent communication skills, keen observational abilities, and a profound understanding of hospice care principles. Your Responsibilities on Our Hospice Nursing Team:
- Provide exemplary bedside care to our patients and their families.
- Educate patients and families on disease processes, self-care strategies, end-of-life care, nutrition, and dietary considerations.
- Collaborate with the Inspire team to ensure comprehensive patient care, functioning as a vital member of our home hospice care team.
- Alleviate symptoms and facilitate a smooth transition at the end of life.
- Maintain accurate documentation, including assessments, physician orders, progress notes, and care plans.
