Position Overview

As a Hospice RN/ RN Case Manager, you will have the opportunity to make a significant difference in the lives of patients and their families during critical moments. We believe in delivering personalized care that enhances comfort and quality of life.

Key Responsibilities

  • Report directly to the Administrator, Executive Director, or Patient Care Manager Senior, maintaining primary oversight and professional management of each patient.
  • Serve as the main point of contact between the physician, patient/family, and hospice team.
  • Deliver direct patient care services to hospice patients while providing supportive care to their families, fostering a trusting and compassionate relationship.
  • Empower caregivers by educating them on how to care for their loved ones.
  • Collaborate with an interdisciplinary team, including Licensed Practical/Vocational Nurses (LPN or LVN), Certified Nursing Assistants (CNA), and support staff to ensure comfort, peace, and dignity for patients.
  • Assess patient and family needs, assisting in the development and implementation of personalized care plans.
  • Maintain accurate clinical records for all assigned patients.
  • Participate in quality assessment and performance improvement initiatives.

Candidate Profile

Essential Qualifications for Hospice Registered Nurse:

  • A compassionate heart: kind, empathetic, and dedicated to patient care.
  • Comfortable providing care in various settings, wherever the patient resides.
  • Exhibit flexibility and composure while supporting individuals during crises.
  • Possess an RN license in the state of employment (or eligibility to obtain).
  • 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology preferred.
  • Additional experience in hospital settings such as Staff RN, Med Surg, PACU, Float pool, ER, critical care, acute care, cardiac, geriatric, admissions, wound care, telemetry, or travel nursing is advantageous.
  • Hold a current driver's license and automobile insurance.
  • Current CPR certification is required.
