Job Overview

POSITION SUMMARY:

The Client Care Coordinator is responsible for providing advocacy, organization, and continuity of evidence-based support for individuals and families in need through a collaborative approach involving triage, assessment, planning, and communication. This role is integral to a multidisciplinary treatment team.

QUALIFICATIONS:

  • A Bachelor's degree in a Human Services related field along with a minimum of 1 year of experience in the mental health sector.
  • For child welfare programs, a minimum of 2 years of experience with at-risk populations and a child welfare certification is required.
  • Proficient in reading, analyzing, and interpreting clinical records, procedures, or governmental regulations. Capable of writing reports and documenting clinical activities, while effectively communicating with managers, patients, service providers, managed care entities, family members, and other stakeholders.

FUNCTIONAL RESPONSIBILITIES INCLUDE:

  • Conducting comprehensive assessments and coordinating community resources tailored to patient needs in accordance with program requirements.
  • Ensuring all clinical and administrative documentation is completed within established timelines as per program guidelines.
  • Maintaining effective communication and fostering cooperative relationships with both internal and external resources.
  • Developing service plans, treatment plans, and/or aftercare plans with clearly defined goals and objectives in line with program requirements.
  • Managing workflow efficiently and maintaining organization as per program standards.
  • Ensuring high-quality clinical documentation is maintained in accordance with program expectations.
  • Performing other duties as assigned or necessary in accordance with program requirements.

COMPLIANCE RESPONSIBILITIES INCLUDE:

  • Adhering to all relevant federal, state, local, and company-maintained standards of compliance, ethics, and policies regarding the administration and delivery of agency services.
  • Meeting deadlines, requirements, and expectations set forth by supervisors, the agency, and the department.
  • Completing administrative tasks and data entry accurately and in a timely manner.

PHYSICAL DEMANDS:

Must possess abilities in reaching, bending, talking, fingering, sitting, carrying, standing, grasping, fine hand coordination, and the ability to remain calm under stress. Capable of reaching with hands and arms, climbing stairs, balancing, stooping, kneeling, crouching, or crawling. Specific vision abilities include close, distance, color, peripheral, depth perception, and the ability to adjust focus. Must be able to lift 50 pounds of dead weight independently. May be required to deliver standing presentations.

WORK ENVIRONMENT:

This role operates in a fast-paced, collaborative team environment emphasizing critical thinking and decision-making with guidance from a program supervisor or manager. The work is conducted in climate-controlled office settings with frequent client interactions. Some programs may necessitate travel within designated areas to facilitate in-home client services or onsite clinical programs across various socioeconomic neighborhoods.
