POSITION SUMMARY

The Care Manager for the Network Initiative within the Homeless Program is responsible for providing advocacy, coordination, and continuity of evidence-based care for individuals and families in need. This role involves a collaborative approach to triage, assessment, planning, and communication with community services.

KEY RESPONSIBILITIES:

  • Conduct comprehensive assessments and coordinate community resources tailored to patient needs in accordance with program standards.
  • Ensure timely completion of all clinical and administrative documentation while maintaining high-quality clinical records as per program guidelines.
  • Foster effective communication and maintain collaborative relationships with both internal and external stakeholders.
  • Develop individualized service plans, treatment strategies, and objectives aligned with program requirements.
  • Maintain organized workflow management and efficiency in accordance with program expectations.

QUALIFICATIONS:

  • Bachelor's degree in a Human Services-related field such as Sociology, Psychology, Criminology, Social Work, Behavioral Health, or Mental Health.
  • Proficient in reading, analyzing, and interpreting clinical documentation, procedures, or regulatory guidelines.
  • Skilled in writing reports and documenting clinical activities, with the ability to communicate effectively with managers, patients, service providers, family members, and other stakeholders.

Monday to Friday, 8:00 AM - 4:30 PM
FULL TIME or as needed
