Compensation Rate: $19.00 Hourly
Overview:
As a key member of the Outreach team, the Family Reunification Case Manager plays a vital role in providing comprehensive case management services to families with children in out-of-home placements. The primary objective is to facilitate a smooth transition back to permanent placement with the parent.
Responsibilities:
- Serve as the main point of contact with the Office of Youth & Family Services (OYFS) caseworker(s), collaborating on the development of transition plans and care recommendations.
- Support and guide children and their families throughout the reunification process, including providing supervised visits when necessary. Ensure access to resources and supportive services.
- Continuously evaluate the goals and service needs of families identified for intensive case management, both pre- and post-reunification.
- Build and maintain community resources to assist families with complex needs, coordinating with various service providers to address challenges related to reunification.
- Participate in OYFS Family Team Conferences and other required meetings, contributing to collaborative discussions.
- Maintain accurate case records and documentation, ensuring data is entered into the agency's data collection system.
- Perform additional duties as assigned to support the mission of the organization.
Qualifications:
To be successful in this role, candidates must meet the following criteria:
- Education: A Master's or Bachelor's degree in Counseling, Psychology, or Human Services, along with relevant experience.
- Technical Skills: Proficiency in Microsoft Word, Excel, and database management is essential.
- Certifications: Preferred qualifications include Crisis Prevention and Intervention training, along with experience in social services and case management.
- Transportation: Reliable transportation, a valid driver's license, and auto insurance are required for travel to various appointments.
- Language Skills: Proficiency in Spanish is preferred.
Work Environment: The work environment is typically moderate in noise level, and physical demands may include lifting or moving items weighing up to 20-25 pounds.
